Membership Form with Connected Event Registration

When new members submit the membership dues form, their email address will be populated in (and hidden by) the Event Registration form. The Event Registration form will charge or waive event fees based on whether or not a registrant’s email address is in the system. All registrants will also automatically be added to the Event Check-In form for use at your event.
Simply add these templates to your Cognito Forms Team-level account. Then customize them for your organization and event to create your own integrated system.
This template uses Lookup fields. For full functionality, you must be on a Team plan or higher and copy its connected form. Learn more about using templates with Lookup fields.
